[ARCHIVED] Deadline Extended for Community Health Improvement Survey

The City of Portland’s Public Health Division encourages all those who live, learn, work and play in Portland to complete the 2021 Community Health Improvement Survey. To help ensure as many members of the public as possible share their input, the deadline to complete the survey has been extended through September 3, 2021.  

The survey is a health opinion survey that gathers information about the health and well-being of Portland and helps identify ways to address those needs and concerns. 

The survey is available online and via telephone until Friday, September 3rd and has been translated into French, Portuguese, Somali, Spanish, and Vietnamese. To complete the survey via telephone, interested participants can call the Portland Public Health Division at 1-207-874-8919.

Residents and interested parties are also encouraged to check out the Community Health Improvement Planning project page to read more about the process and sign up for project alerts.
